Dental Clinic Accreditation Eligibility Criteria

Organizations are considered for review by the CRDA on an individual basis. An organization is eligible for an accreditation review by the CRDA if the organization meets the following criteria.

The organization:

  • Has been providing dental care services for at least six months before the on-site review.
  • Is a formally organized and legally constituted organization that primarily provides dental care services.
  • Is in compliance with applicable laws and regulations.
  • Is licensed by the nation of Costa Rica.
  • Provides dental care services under the direction of a licensed Doctor of Dental Surgery (DDS).
  • Shares the facilities, equipment, business management, and records involved in patient care among the members of the organization.
  • Submits the completed, signed Application for Review, all supporting documents, and application fee in advance of the survey.
  • Pays the appropriate fees in accordance with CRDA policies; see Review Fees.
  • Acts in good faith in providing complete and accurate information during the accreditation process and during a term of accreditation.

CRDA reserves the right to reject any application. If a review is conducted and the CRDA determines that the standards applied do not allow CRDA to reach an accreditation decision, the review will be billed as a consultation and no accreditation decision will be made. Fees for such a consultation will not be refunded.



How to Achieve Accreditation for your Costa Rica Dental Clinic

CRDA offers a three-year dental clinic accreditation program that certifies that an approved facility has met internationally recognized standards for safety, quality control and patient satisfaction. This dental clinic accreditation program exists to ensure optimal patient safety. Costa Rica dental clinics that have been accredited by the Costa Rica Dental Association have met standards that ultimately result in the delivery of safe dental care. Patient safety is our top priority. Accredited Costa Rica dental clinics must meet all accreditation standards. If a dental clinic fails to meet any standard, it must correct the concern before receiving accreditation. Patients that choose CRDA accredited dental clinics in Costa Rica can be confident that their chosen clinic meets the highest level of international standards for patient safety and care.



Accreditation Process for Costa Rica Dental Clinics

New Applicants

Below is a list of documentation that must be completed for the CRDA Dental Accreditation Application. Please email completed documentation to CRDA: info@costaricadentalassociation.org

  • Application Form with Payment
  • Dental Clinic Floor Plan
  • List of All Staff with Appropriate Titles and Job Responsibilities
  • Copies of Medical/Dental License for Each Practicing Dentist/Specialist
  • Authorization to Release Information Form (Forms must be signed by each staff member)
  • Anesthesiology Provider Form (if applicable)

Once the documentation is received by CRDA, it will be reviewed. Applicants will be contacted within 30 days if additional paperwork is needed.


Self-Review: Accreditation Renewal: Year 2 and 3

Below is a list of documentation that must be completed for the CRDA Dental Self Review. Please email completed documentation to CRDA: info@costaricadentalassociation.org

  • List of All Staff with Appropriate Titles and Job Responsibilities
  • Copies of Medical/Dental License for Each Practicing Dentist/Specialist
  • Authorization to Release Information Form (Forms must be signed by each staff member)
  • Anesthesiology Provider Form (if applicable)
  • Add a Dentist Form (if applicable)
  • Anesthesiology Provider Form (if applicable)
  • Self-Review Score Card
  • CRDA Dental Standards and Checklist Manual

Once the documentation is received by CRDA, it will be reviewed. Applicants will be contacted within 30 days with a Correction Plan and 30 days for review of new evidence. A new digital certificate will be sent at no charge.


Re-Application Process: Year 4 – Process to be Completed Before Year 4 Begins

Below is a list of documentation that must be completed for the CRDA Year 4 Accreditation Process. Please email completed documentation to CRDA: info@costaricadentalassociation.org

  • Application Form with Payment
  • Dental Clinic Floor Plan
  • List of All Staff with Appropriate Titles and Job Responsibilities
  • Copies of Medical/Dental License for Each Practicing Dentist/Specialist
  • Authorization to Release Information Form (Forms must be signed by each staff member)
  • Anesthesiology Provider Form (if applicable)
  • Add a Dentist Form (if applicable)
  • Self-Review Score Card

Once the documentation is received by CRDA, it will be reviewed. Applicants will be contacted within 30 days if additional paperwork is needed. It is your responsibility to contact the reviewing agent assigned to your organization to schedule a date for your on-site review.
