Dental Clinic Certification Fees
The scope of a certification review is determined by CRDA staff based on review of the application and supporting documentation. Questions regarding the scope of an on-site review should be directed to CRDA before the on-site review is scheduled.

Typical dental clinic certification fees are as follows:
Dental Clinic Size: 1-5 Employees

- Year 2 and Beyond:
- $750.00 per year
Dental Clinic Size: 6-10 Employees
- Year 1:
- $1500.00
- Year 2 and Beyond:
- $1000.00 per year
Dental Clinic Size: 11 or more Employees
- Year 1:
- $2000.00
- Year 2 and Beyond:
- $1500.00 per year
Individual Dentist/Specialist Certification Fees
$250 per dentist/year
Discounts are offered to dental clinics that pay for individual dentist certifications.
Dental Laboratory Certification Fees
Dental Lab Size: 6-10 Employees

- $1500.00
- Year 2 and Beyond:
- $1000.00 per year
All pricing is subject to change. Organizations that are not prepared for the site visit and who fail the certification process, must apply again and pay a partial or full fee for an additional site visit. The decision of the CRDA representative is final on all matters.
The survey fee is based in information obtained from the organization’s Application for Review and supporting documentation. Factors considered in determining price include the size, type and range of services provided by the organization. An invoice will be sent when the application is determined to be complete. The review fee must be paid no later than 14 calendar days from the invoice date. Failure to pay the review fee in advance will result in cancellation of the CRDA accreditation review.
Scheduling:
On-site review dates are determined by the CRDA in cooperation with the organization requesting the review. Once an on-site review has been scheduled, the CRDA will confirm the date of the review, the name(s) of the reviewers, the review schedule, and other information about the review
